Convince my doctor to give me surgery?

I have tmd and my right jaw joint is assymmetrical with my left, it hurts all the time now. got an mri and the cartilage is worn to the bone. my doctor said try physical therapy for 6-9 more monthd and then see about other options. i just waited 4 months using a jaw splint. I am going to college in september. summer is prime for surgery. otherwise in about 4 months Im gonna end up being on percocet full time. how can I convince my doc to give me surgery?
